Diagnostic Testing Requisitions
Our physicians provide requisitions for bloodwork, X-ray, MRI, CT scan, ultrasound, bone density scan, pregnancy test, mammogram, and many other diagnostic tests. Take the completed form to the lab or imaging centre of your choice, and we’ll contact you when results are in.
